Dr. Jho's endoscopic endonasal removal of chordomas via a nostril without a skin incision
Chordomas are locally malignant tumors that can develop at the clivus or the spine (especially at the upper cervical or the lumbosacral spine).  Treatments consist of surgical removal followed by radiation therapy.  Dr. Jho has developed minimally invasive surgical techniques for clival chordomas or chondrosarcomas.  Surgery is performed with an endoscope, which is passed in front of the tumor through a nostril.  Unlike conventional radical extensive skull base surgery which often requires face splitting or radical cranial opening, Dr. Jho's minimally invasive surgery is carried out through a nostril without any skin incisions.  Thus, postoperative recovery is easy and quick.  Hospital stay is often only overnight.

Figure 1: MR scans, axial (A) and sagittal views (B), show a large chordoma at the clivus and posterior cranial fossa preoperatively.  The tumor was totally removed with an endoscope through a nostril.  The patient's hospital stay was overnight. Postoperative MR scans, axial (C) and sagittal views (D), demonstrate complete removal of the tumor.  However, the tumor removal site was treated with gamma knife radiosurgery due to of the malignant nature of tumor.
